Iron and Vitamin D Deficiency Hair Fall: Signs, Tests and Diet Tips

If your hair has been thinning slowly and you also feel tired, breathless or low on energy, your diet and blood levels may be part of the story. Low iron and low vitamin D are two of the most common nutritional problems among women in Pakistan, and both are linked with hair fall. The good news: a simple blood test can tell you where you stand.

Quick answer

Low iron stores (measured as ferritin) are linked with increased hair shedding, especially in women. Low vitamin D has also been linked with hair loss in research, although the evidence is less clear. If you have ongoing hair fall, ask your doctor for a CBC, ferritin and vitamin D test. Correct any deficiency under medical advice, eat iron-rich foods, and support your scalp while hair regrows.

How common are these deficiencies in Pakistan?

According to Pakistan's National Nutrition Survey 2018:

  • 41.7% of women of reproductive age are anaemic
  • 18.2% have iron deficiency anaemia
  • 79.7% are deficient in vitamin D, about four out of five women

That makes these two of the first things worth checking when a woman in Pakistan notices unexplained hair fall.

How low iron affects hair

Hair follicles are some of the fastest-growing cells in the body and need a steady supply of oxygen and nutrients. When iron stores run low, the body prioritises vital organs, and more hairs can move into the shedding phase. This usually shows as diffuse thinning across the scalp, not bald patches. Heavy periods, pregnancy, breastfeeding and low-meat diets all raise the risk.

How low vitamin D may affect hair

Vitamin D plays a role in the normal hair growth cycle, and several studies have found lower vitamin D levels in people with hair loss. Research has not yet proven that vitamin D supplements regrow hair on their own, so treat it as one part of the picture rather than a cure. Indoor lifestyles, covering clothing and limited sun exposure make low vitamin D very common.

Signs that deficiency may be involved

Low iron Low vitamin D
Tiredness and weakness Tiredness and low mood
Pale skin, breathlessness on stairs Bone or back pain
Brittle nails, cold hands and feet Muscle aches or weakness
Diffuse hair shedding Diffuse hair shedding

Tests to ask for

  • CBC (complete blood count) to check for anaemia
  • Ferritin to measure your iron stores; it can be low even when haemoglobin is normal
  • 25-hydroxy vitamin D
  • TSH if your doctor suspects a thyroid problem (see PCOS and thyroid hair fall)

Do not self-prescribe high-dose supplements

Too much iron can upset the stomach and, over time, build up in the body. Very high doses of vitamin D can also be harmful. Take supplements only at the dose your doctor recommends, and re-check your levels as advised.

Foods that help

For iron:

  • Daal, chana, lobia and rajma
  • Palak and other leafy greens
  • Beef, mutton, chicken, kaleji and eggs
  • Pair iron-rich meals with vitamin C foods such as lemon, amrood, kinnu or tomatoes to improve absorption
  • Avoid drinking tea or coffee with meals, as they can reduce iron absorption; leave a gap of about an hour

For vitamin D:

  • Safe sun exposure on arms and face as your doctor advises
  • Eggs, oily fish and fortified milk or cooking oil

Support your scalp while levels recover

Correcting a deficiency is what lets hair recover, but a good routine helps your scalp in the meantime. Keep the scalp clean with a sulfate-free clarifying shampoo, use a leave-on night serum on thinning areas, and protect hair during the day with a light hair growth spray. New hair grows about 1 cm a month, so give it at least 3 months.

Frequently asked questions

Can low iron cause hair fall?

Yes. Low iron stores are linked with increased shedding, particularly in women. A ferritin test shows your iron stores more accurately than haemoglobin alone.

Does vitamin D deficiency cause hair loss?

Low vitamin D has been linked with hair loss in studies, but supplements alone are not proven to regrow hair. It is still worth checking and correcting if it is low.

How long after fixing iron levels does hair grow back?

Shedding often settles over a few months once iron stores improve. Visible regrowth takes longer because hair grows slowly.

Which blood test should I do for hair fall?

Common first tests are CBC, ferritin, vitamin D and TSH. Your doctor may add others based on your symptoms.
